Description

The Peer To Peer Empowerment Group proposes to Implement Peer Base Mentorship Program. The purpose is to assure youth/young adults, and family a safe warm environment to receive information regarding their missing loved one, classes to uplift and motivate, Peer Support Specialist on site when participants are being released, The “you are not alone” concept, to gain trust amongst peers, a project designed tailored to their needs and empower participant with the concept of second chances as a new beginning. Target population includes social and economically underserved/disadvantage communities. LGBTQ+, Gender Nonconforming, are 4 time likely to be the victim of violent crimes (FBI UCR). Immigrants, Refugees/Asylees, Ethnic/racial minorities, Black/African American, Hispanic/Latino, Tribal, persons living with HIV/AIDs, and all residents who are determined to be Low Income level to Poverty. Participants are youth and young adults. Young adults are males and female, and considering the probability a percentage may be parents, parents included as it relates to aged out of Foster Care, at risk youth, who may have criminal history and/or victims of violence, separated from family and known environment at no fault of their own. The primary population served by such pilot programs will include adolescents and youth/young adults transitioning out of foster care who have experienced a history of foster care involvement, child poverty, child abuse or neglect, human trafficking, juvenile justice system involvement, substance use or misuse, or gang involvement. An established relationship with Community Gatekeepers (Faith Base), First Responders such as: Hospitals, Law Enforcement and Community base Organizations. We gaveled representatives from each as a Discovery Advisory Board who is willing to activate and motivate the youth/young adults with realistic alternatives, and if applicable adopt the Cardiff model (Who, What, When, Where, and Why) the model is about solutions and tracking.
