Description

ConnectAbility was founded in 2016 as an initiative of the Warm Springs Foundation to assist people who had experienced a catastrophic injury such as a Traumatic Brain Injury, Spinal Cord Injury, Amputation, Stroke or other similar debilitating injury or disability to regain their independence, improve quality of life and move forward in their “new normal”. 

We offer Care Coordination – navigating individuals through complex medical and insurance systems, connecting them to community resources, and in many cases, assisting with financial resources when there is no other source of payment.  In addition, we offer multiple support groups for individuals and their families with various disabilities, peer mentoring, and caregiver supports such as counseling for a loved one whose role is now as a caretaker. 

All of these services are free of charge to a person of any age, gender, religion, etc.  ConnectAbility became a standalone 501c3 in 2021 with a projected budget for this year, just above $1M. Since our inception in 2016, we have served over 1000 individuals through Care Coordination, and over 1,700 individuals in our other service offerings such as support groups and social events held for individuals with the disability and their family members.

ConnectAbility is asking for your support for our Crisis Fund.  Our Crisis Fund is accessed by our Care Coordinators to assist individuals financially when there is no other payment source.  We pay for wheelchair ramps and installation, accessible home modifications (door widening, restroom accessibility, etc.), prescription medication, physician copays, transportation costs for rides to physician appointments, prosthetic assistance for amputees, durable medical equipment and supplies, Housing/Utility assistance, Food, and more! 

If you are unable to accommodate a monetary request for our Crisis Fund, please consider donating either catering or goods/services/Gift Cards that we could use as special gifts to our clients with disabilities during times of Crisis and/or as Raffle/Door Prizes at one of our Client Appreciation Events.

Our next event is our annual Holiday Social on December 2, 2023.  We typically have an attendance of about 100 people who have been directly affected by a life-altering disability due to sudden, traumatic injury, and the donations we receive from companies like yours are wonderful gifts to present to our clients with disabilities during the often difficult holiday season.

We are writing to ask for a donation to assist us in fulfilling this essential mission for people with life-altering disabilities and their families.  These types of injuries or disabling conditions can happen to anyone, anytime, changing lives forever.  We believe at ConnectAbility that finding help shouldn’t be the biggest obstacle someone should have to face.
