Dear Sir/Madam:

I am writing on behalf of the Parent Teacher Association (PTA) for Norman J. Levy Lakeside, located in Merrick, New York.  On May 15th, our school will be hosting our annual PTA End of Year Dinner.  At this event, we will be raffling off baskets to raise money for our school’s PTA, which will benefit childrens’ activities, school trips, and educational assemblies. We are asking merchants for donations in the form of merchandise, services, or gift certificates.

All businesses and donations will be promoted throughout our school.  Therefore, we ask that all donations be mailed or can be picked up by April 15th so we can assemble an appropriate donation list.  Please feel free to mail donations to:
