Golf Tournament for Lung Transplant Support Organization Inc.May 15, 2021 from 7:00 AM to 1:00 PM
We are the Lung Transplant Support Organization and we were started here in Phoenix in 2008, by 2 lung transplant recipients and their care givers. We are an IRS determined 501.c.3, (ein 26-2876893) that helps those who might otherwise not be able to afford a lung transplant receive one. We provide assistance with no discrimination other than financial need. We have no employees, we are all volunteers.
The mission of the Lung Transplant Support Organization is to provide assistance to fellow lung transplant recipients, including but not limited to:
Educating and promoting donor awareness.
Educating and supporting transplant recipients throughout the transplant process.
Providing limited financial assistance to fellow lung transplant recipients (supplementing their current medical and pharmaceutical benefits and / or available assistance programs).
It is our goal to assist in making the transplant journey a positive and easy one for the transplant recipient and their caregivers.

We are asking you for a gift in kind for our 12th Annual, “Breath of Hope” charity golf tournament. It is being held on Saturday, May 15, 2021 at: The Legacy Golf Club, 6806 S. 32nd Street, Phoenix, AZ. The shotgun start begins at 7:30 AM and it ends with lunch and a silent auction on completion of play in the early afternoon. It draws roughly 140 golfers and 20 to 30 other luncheon only attendees; who participate in raffles and the silent auction of the goods in kind we receive.
I can be contacted reference any questions you would like us to answer for you. I have included a copy of our IRS 501.c.3 determination letter and a gift in kind receipt form for your use. I have also attached a sponsorship form, should you desire to become more involved with our group. We are hoping that you will be willing to provide us with a Stay at your Hotel. The Stay is very popular in the area and would be a major hit at the tournament as a raffle item, or more probably, in the silent auction.
