Siskiyou Community Health Center is a Federally Qualified Health Center (FQHC), and 501(c)(3). We operate multiple clinics in rural Josephine County (Southern Oregon), offering primary care, dental, urgent care, and pharmacy services. Our Outreach programs offer family support services (for families with children ages 0-5 with high-risk challenges facing severe economic or health crises) in both Jackson and Josephine Counties and mobile dental screenings in local schools. We also operate five school-based health centers.

Our biggest fundraiser each year is Tee Off For Kids, now in its 16th year. This charity golf tournament helps fund some of the 2,500+ dental screenings conducted each year in collaboration with our local elementary schools. This fundraiser also allows the family support programs to host holiday gatherings that provide Christmas presents and warm winter clothing and accessories for children.

We are asking today for your support of our Tee Off For Kids tournament and your support in serving some of the most vulnerable children and families in Southern Oregon. You can do this by donating to our raffle and auction at the September 12th event. We will mention your donation on our social media channels, clinic television screens, and at the event itself.
