La Clínica de La Raza (La Clínica) a non-profit, community health clinic with 35 sites throughout Alameda, Contra Costa, and Solano counties. La Clínica provides a full range of services, including primary medical care, dental, vision, mental health, and health education to uninsured and underserved families. We believe high quality and accessible healthcare should be available to everyone, regardless of their race or ability to pay.

La Clínica’s Transitions Clinic (TC) was implemented to serve formerly incarcerated community members with chronic medical conditions. TC provides access to medical, behavioral, and substance abuse treatment, as well as other social resources that can help our patients incorporate into a self-sufficient lifestyle.

We invite you to support our clinic by providing a tax-deductible donation of goods or services to our Transitions Clinic Annual Fundraiser raffle in December 2021.

Your support will help make a huge impact on our community and the people we serve, helping to provide resources such as birth certificates, driver’s license, work apparel, emergency hotel vouchers, and monthly bus passes or gas cards. Donors will be acknowledged in our fundraiser communications and our social media.
