Buffalo Prenatal Perinatal Network empowers women, fathers, and families to take charge of bettering their family health and well-being by providing education, support, advocacy, and connection to resources and services in WNY. The work of Buffalo Prenatal Perinatal Network is dedicated to honoring the dignity and capacity of individuals and communities and in promoting health equity services within the 28 highest risk zip code areas of Erie County in Buffalo, N.Y.  The agency serves approximately 1,000 people each year in Buffalo, N.Y. 

Healthy Families Buffalo (Buffalo Home Visiting) is designed to increase self-sufficiency while decreasing child abuse and neglect.  The program matches parents with knowledgeable and caring workers who provide information and support during pregnancy and early childhood. Services include helping families access community resources and services, educating families on parenting and child development, connecting families with medical providers, and assessing children for developmental delays.   Services are received in the home and at no cost to the family.

We help/assist in:-Ensure that families’ basic needs are met (help is given to families who need services from food pantries, public assistance, WIC, housing assistance, etc.)

-Provide families with a family support worker who will visit them in their home once a week to provide educational information and resources for up to 5 years.

-Promoting a positive parent/child in Healthy Families Buffalo (Buffalo Home Visiting). It is designed to increase self-sufficiency while decreasing child abuse and neglect.  The program matches parents with knowledgeable and caring workers who provide information and support during pregnancy and early childhood. Services include helping families access community resources and services, educating families on parenting and child development, connecting families with medical providers, and assessing children for developmental delays.   Services are received in the home and at no cost to the family.

 -Interactions by educating families on child development and by providing activities to help them enhance their child’s growth and development.

-Strengthen the family support system by linking them to appropriate programs and agencies.

-Empower and educate families to enable self-advocacy.

This program is a part of a nationwide initiative called Healthy Families America (HFA), The Buffalo Home Visiting Program is based on over two decades of research and experience with other home visitor programs and is part of the statewide Healthy Families New York System funded by the office of Childhood and Family Services.

The Community Health Worker Program (Maternal Infant Community Health Collaborative) has a long history of helping clients navigate the services they need and how to access them.

We assist in:

-seeking a primary home/doctor, WIC, a Dentist, Medical /Health insurance, etc.

-scheduling the clients and clients’ children’s appointments.

-help with transportation needs, translation, and advocacy.

-aid in obtaining family planning, birth control, and a doctor who can address current health concerns.

- help find other services in the community.

-We provide education on health topics and private supportive visits in the comfort of the clients’ home.

-We provide Nutrition, Breastfeeding, and Empowerment support groups that clients can join.

The Buffalo Fatherhood Initiative Program empowers healthy father and male involvement for children and families while providing tools and resources to a better future for our children and our community – one father at a time.

-Personal Development & mentoring for young adult males and fathers aged 18 – 26 years

-Parenting education and support through our 13-week Nurturing Fathers Program

-Promotion of economic stability through our 9-week personal financial education program
