Community Nurse Home Care is an independent, not-for-profit agency dedicated to providing a continuum of quality home care and hospice services to the communities of Southeastern Massachusetts since 1916. Our licensed professional clinicians, which include nurses, physical and occupational therapists, home health aides, social workers, and a dietician, are highly skilled, compassionate caregivers who create and manage comprehensive recovery plans so that patients may remain in the comfort of their own homes. We put patients first and ensure our patients get optimal care with high patient satisfaction. The Centers for Medicare and Medicaid Services has awarded Community Nurse Home Care a 5-Star rating for quality and patient satisfaction and our hospice program has a 5-Star rating for patient-caregiver satisfaction. Our generous donors and fundraising events allow us to provide services such as dietician consultations and to utilize our care transition program that is not reimbursed by insurance. We are also able to purchase durable medical equipment that is needed to keep patients safe at home such as bed rails or shower chairs that are also not reimbursed by insurance. Our Hospice Program offers patients care at home who are nearing the end of life with compassion and respect. We also offer spiritual and emotional support for the patient's family.
