Project Paz Promoting Alma & Zalud provides navigation services to individuals with severe mental illness and chronic illnesses that are residents of rural communities in Brooks, Duval, Jim Wells, and Kleberg Counties. The intervrention are client directed classes that manage chronic illnesses, health and wellness and substance use in collaboration with existing community partners. Navigation services include transportation that supports adherence to follow up care, chronic disease management classes and reduce social isolation by chronic illnesses. Project Paz Alma & Zalud measure's improvement in depression, access to care and quality of life.
Our Real STx Community Clubhous is a community cent where adults with Mental Health Conditions can reduce isolation, learn, and grow as members of the community.
Here, you're not a patient, you're a unique and important member of our community. We believe every has a gift or a special talent no matter of their mental illness. Clubhouse impowers members to show case thier talents, and facilitates access to impower our members to show case their forgotten abilities.
