SAGE Eldercare, Inc. is a unique community resource that offers a comprehensive array of programs and services for older adults and caregivers, promoting independence and quality of life. SAGE Eldercare, Inc. will be the leading provider of innovative services and programs that meet the changing needs of older adults and families.
SAGE Eldercare's programs include Meals on Wheels; Spend-A-Day Adult Day Health Center; InfoCare referral service - providing free information, medical assessments and social work support; HomeCare by certified health aides and nurses; Personalized care transitions support to older adults and their caregivers; SHIP: State Health Insurance Assistance Program; Caregiver Support Groups; and Support services including Grocery Shopping, Errand Service and an Emergency Food Pantry. Annually, the nonprofit serves more than 7,000 older adults and their families in Union, Essex, Morris, and Somerset Counties in NJ and beyond.
