The Siouxland Community Health Foundation is the fund development entity of Siouxland Community Health Center (SCHC), a federally qualified health center whose mission is inspiring and empowering the community through health equity. Siouxland Community Health Center is located in Sioux City, Iowa and is part of the Sioux City metropolitan statistical area (MSA) for northwest Iowa, northeast Nebraska, and southeast South Dakota. Founded in 1991, SCHC delivers health care services to over 38,000 patients annually, many of whom are underserved and vulnerable to health disparities. Services offered include primary medical care, dental, and behavioral health services, prenatal care, onsite pharmacy, and other services that bring care into a single setting and reduces access barriers. To assist patients in need, the Siouxland Community Health Foundation has various funds established that assist patients with basic needs such as clothing, transportation, prescriptions, essentials for infant care, a mobile shower for the homeless, and other funds that support additional areas of need that often result from social determinants of health.
