Achievable Health (formerly The Achievable Foundation)is a nonprofit community-based organization whose mission is to provide high quality, integrated health care to individuals with intellectual and developmental disabilities (I/DD), their families, and other vulnerable populations. Achievable Health was founded in 1996 to offer safety net supports to individuals with I/DD and their families when no other funding source was available. To address the health disparities and barriers to healthcare experienced by individuals with I/DD, Achievable Health launched its own community clinic in September 2013 with services specifically tailored to this underserved population. This health center was awarded designation as an Federally Qualified Health Center (FQHC) in November 2013. Our operations now revolve fully around our health center. Through our health center we aim to provide high quality, integrated, equitable, whole-person care to over 2,100 community members annually. Our target population includes underserved patients from low-income households, and those with I/DD, residing in 28 adjacent zip codes in Los Angeles County. We serve patients of all backgrounds, ethnicities, ages, genders and sexual orientation. Our activities include direct primary care, mental health and enabling services; collaborations on workforce development programs; and participation in advocacy work to increase awareness of the healthcare needs of those with I/DD and medical complexities. Our overall goal is to help eliminate health disparities and facilitate better health, independence and quality of life for patients. Our vision is to change the face of healthcare for our vulnerable communities.
