Yolo Community Care Continuum (YCCC) is a non-profit agency that provides an array of community-based programs for people experiencing a psychiatric disability. YCCC has been providing services in Northern California since 1979. Each service is designed to meet the individual needs at specific points in the recovery process. Twenty-four-hour supervision is offered in three residential programs. Supported housing offers an opportunity to live more independently in the community while receiving the support needed to be successful. YCCC's homeless outreach program assists individuals to get a psychiatric evaluation, substance abuse treatment, government benefit filings and help with accessing medications through the Patient Assistance Program (PAP).
