The SAM's Sweet Heart Memorial Foundation, honors the lives of Alissa and Luke Bartel's two children, Avery and Samuel, who both passed away before the age of one with complex terminal medical diagnoses. Their children's legacy lives on through the foundation that provides assistance for the difficulties that arise to families who are affected by complex medical conditions. 

Over the last 8 years, SAM's Sweet Heart Memorial foundation has raised and donated $107,000 to local families. Through our annual golf outing and luncheon event, we've been able to help provide support for consulting, hospice, and direct family grants to ease the financial burden families experience due to these conditions. 

On Saturday, June 3rd, we will be hosting our 8th annual SAM's Sweet Heart Golf Outing and Luncheon event at Macktown Golf Course and The Venetian Club in the Rockford, IL area. This event includes a silent auction to help further our fundraising efforts so that we can continue to grow our family grant program and serve more families in our local community.
