The mission of Aging Care Connections (ACC) is to enrich the quality of life in our community by providing older adults and their families with guidance and support that enhances the ability to age well. We are advisors on all matters related to the issues facing older adults as they age. The funds raised at our annual fundraising Gala will support our programs, which range from Adult Protective Services to Grandparents Raising Grandchildren. We provide the following services: Adult Protection; Aging Well Neighborhood; Benefits Assistance; Caregiver Support; Community Care Program; Comprehensive Care Coordination; Home Delivered and Congregate Meals; Information and Assistance and Transitional Care.
