Mission

Empowering people with disabilities to improve their housing stability, achieve better health, and reclaim their lives.

Impact

  • 550 formerly homeless people with disabilities are living independently in every neighborhood in Philadelphia.
  • 85% of those who were previously chronically homeless retained their housing.
  • Over 80% of Pathways participants are engaged in both primary and psychiatric care.
  • More than 65% of participants with opioid use disorder seek treatment within six months of being housed.
  • The Integrated Care Clinic provided services for 300 unduplicated participants in FY20
  • Since its founding, the Philadelphia Furniture Bank has furnished 3,460 homes- that’s 7,249 adults and children who are now sleeping in a bed of their own.  
     

History

Founded in 2008 to bring its evidence-based Housing First model to Philadelphia Pathways works to help end chronic homelessness. As an alternative to emergency shelter and transitional housing, our Housing First model is simple: provide housing without preconditions, and then address issues around mental health, addiction, medical care, and education to welcome people back into the community. We believe that housing is a basic human right, and everyone deserves a place to call home.

In late 2014, the Philadelphia Furniture Bank was created as a centralized resource for furniture for human service organizations across Philadelphia. Member agencies utilize the warehouse for clients who then choose their furniture as they move out of homelessness. Each individual or family receives a full home’s worth of furniture, including brand new bedding.

 

Core Services

Provide homes and comprehensive wrap around services for people who have experienced chronic homelessness and suffer from serious mental illness, substance use disorder, and/or multiple other disabilities.

Restoring health by providing low barrier primary care services, medication management, and the coordination of psychiatric and addictions services to our program participants, while respecting harm reduction as a method for achieving goals.

Reclaiming lives by promoting community inclusion and helping participants to be a part of the community and be valued for their own uniqueness and abilities, just like everyone else.
