Sherbourne Health is a dynamic provider of integrated health services, community programs and capacity-building initiatives that enable people and diverse communities to achieve wellness. As an urban health agency in downtown Toronto, Sherbourne provides primary care and chronic disease management, mental health services, health promotion and education, and outreach and social supports. Sherbourne’s doors are open to everyone, with a focus on marginalized and racialized populations whose complex needs are often not met by traditional health care including 2SLGBTQ communities, people experiencing poverty and/or homelessness, and refugees and newcomers to Canada. Sherbourne combines high quality clinical care with responsive, culturally appropriate community development programs that bring low-barrier services to people in the surrounding communities who need it most.

Sherbourne Health programs and services include:

- SOY (Supporting Our Youth), an award-winning and innovative community development program featuring services and programming centered on supporting the health and well-being goals of 2SLGBTQ youth.

- ARC (Acute Respite Care) offers short-term 24/7 medical respite care for homeless, under-housed or socially isolated people in need of a safe place to rest or recover from an acute medical condition, illness or injury.

- Health Bus, our mobile health unit which increases access to primary care and mental health services outside of normal business hours for individuals who experience homelessness or are under-housed.

- WINK (Women In Need “Klinik”), a weekly drop-in that offers accessible, low-barrier services to women and trans women who are homeless, sex workers and those living in poverty.

- Rainbow Health Ontario, a provincial program that transforms health care for LGBTQ people through research and public policy development and by educating organizations, health professionals and the general population on the health issues and needs of the LGBTQ community.

- Trans Health Care which provides: surgery planning visits and referrals for transition-related surgeries (TRS); 24/7 care for those in need of a safe place to recuperate from post-surgical recovery and TRS care; social drop-in groups for adults and youth who identify as trans; and peer-to-peer support groups for trans men who are considering, waiting for, or have undergone TRS.

- Sherbourne Health also provides: Diabetes Education Program; Toronto Community Hep C Program; HIV Clinic and Care; Cancer Prevention and Support; Food and Nutrition Programs; and Mental Health and Counselling Services with a focus on homeless and under-housed people, the LGBT2SQ community and newcomers to Canada.
