The Achievable Foundation (Achievable) is a nonprofit whose mission is to provide high quality, integrated health care to individuals with intellectual and developmental disabilities (I/DD), their families, and other vulnerable populations. I/DD can include autism, cerebral palsy, intellectual disability, Down syndrome and similar disabilities. Our vision is to change the face of healthcare for our most vulnerable community members.
Achievable's health center was the first-of-its-kind FQHC with services specifically tailored to individuals with I/DD and their families. Although originally established to serve those with I/DD, over the years Achievable has become intimately familiar with the healthcare disparities that exist within vulnerable populations. As a result, Achievable is committed to becoming the medical home of choice for our patients of all abilities. The health center is currently Achievable's main program.
